Job summary

Omega Hospice Care LLC is seeking a Registered Nurse to provide admissions and comprehensive nursing care in patients' homes within the North Dallas area. The role coordinates with the interdisciplinary team, families, and referring agencies to ensure seamless, compassionate end-of-life care.

Responsibilities include patient assessments, care planning, symptom management, thorough documentation, and participation in team meetings.

Qualifications

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current State license as a Registered Nurse; Current driver’s license.
Two (2) year’s experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
CPR certified.
Familiar with electronic health records documentation software (Kantime).
Reliable transportation and valid auto liability insurance.

Responsibilities

Conduct admissions and administer nursing care for patients in their place of residence.
Coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and assume responsibility for coordination of care.
Perform patient assessments and develop individualized care plans with the hospice team.
Utilize Nursing Assessment and LCD criteria to identify needs.
Administer medications and monitor pain and symptom management.
Collaborate with hospice team and participate in regular team meetings.
Document assessments, interventions, medications, patient responses, and changes in condition.
On-call rotation and after-hours care as needed.
Provide family education and support about the dying process.
