Job summary

Alpha Omega Hospice is seeking a Full Time Hospice RN Case Manager to join our compassionate team in Fort Worth, TX. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through the hospice journey.

The ideal candidate holds an active RN license with at least 1 year of RN experience in acute care; EMR proficiency is required. The role offers a supportive, growth‑oriented environment with a comprehensive benefits package.

Qualifications

Hold current unencumbered RN license.
Minimum 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
Proficient with EMR and computer systems.

Responsibilities

Respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess client eligibility for hospice services.
Provide direct nursing care to maximize comfort and quality of life.
Deliver nursing with proven principles to ease pain and distress.
Educate patients and families to create personalized care plans.
Share call duties as part of the team.
