Job summary

Omega Hospice Care is seeking a Registered Nurse to perform admissions and deliver nursing care for patients in their homes, coordinating with the interdisciplinary team to ensure comprehensive, compassionate care. You will assess patients, manage symptoms, and update care plans in collaboration with families and care providers.

Ideal candidates hold an RN license, CPR certification, and a minimum of two years nursing experience including hospice exposure.

Qualifications

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current State license as a Registered Nurse; Current driver’s license.
Two (2) year’s experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice experience.
CPR certification required.
Reliable transportation and valid auto liability insurance.
Familiar with electronic health records software (Kantime).

Responsibilities

Perform admissions and administer nursing care for patients in their place of residence.
Coordinate care with Interdisciplinary Group/Team, patient/family, and referring agency.
Develop and update individualized care plans with the hospice team.
Provide comprehensive patient assessment and symptom management.
Collaborate with doctors, social workers, chaplains, therapists, and volunteers.
Maintain accurate patient records and timely documentation.
