Job summary

Bristol Hospice is seeking a compassionate Registered Nurse (RN) to join our Merced, CA team. The role focuses on comprehensive patient/family assessments and delivering professional nursing care within the hospice framework.

The position requires acute care experience, current California licensure, and willingness to travel within the Bristol Hospice service area. Flexible scheduling and PRN hourly compensation are offered in a patient-centered environment.

Qualifications

• Minimum one year acute care experience in an institutional setting.
• One year home care experience preferred.
• Current licensure in the state.
• Understanding hospice philosophy and needs of the terminally ill.
• Flexible in work hours.
• Willingness to maintain knowledge of information systems and software.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness.
• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status and write/initiate plan of care.
• Re-evaluate patient and family needs; revise plan of care as necessary.
• Initiate and revise plan of care as patient status changes.
• Use health data to determine nursing diagnoses.
• Develop a care plan with goals based on nursing diagnoses.
• Include patient and family in planning.
• Record pain/symptom management and outcomes.
• Communicate with physician and obtain orders as required.
• Teach patient and family self-care techniques.
• Work with interdisciplinary team.
• All other duties as assigned.
