Job summary

Bristol Hospice is seeking a compassionate Registered Nurse in Merced, CA to provide comprehensive hospice care and assessments, working PRN hourly shifts.

The role emphasizes patient and family needs, collaborative care with interdisciplinary team, and adherence to Bristol's compassionate values.

Shifts include evenings and weekends; salary is $48.00-$49.00 per hour. Benefits and travel within service area may apply.

Qualifications

• Must have at least one year of acute care experience in an institutional setting.
• One year of home care experience preferred.
• Must have current licensure in the state.
• Must understand hospice philosophy and needs of the terminally ill.
• Must be flexible in work hours.
• Must demonstrate nursing skills per competency checklist.
• Must maintain comprehensive working knowledge of information systems and software programs.
• Ability to travel throughout the Bristol Hospice Service Area.

Responsibilities

• Complete initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es).
• Provide professional nursing care by utilizing all elements of the nursing process.
• Assess and evaluate patient’s status by writing and initiating plan of care.
• Regularly re-evaluate patient and family/caregiver needs.
• Participate in revising the plan of care as necessary.
• Initiate the plan of care and make revisions as patient status changes.
• Use health assessment data to determine nursing diagnosis.
• Develop a care plan with goals based on nursing diagnosis.
• Include patient and family in planning process.
• Record pain/symptom management and changes/outcomes.
• Communicate with physician regarding patient’s needs and report changes.
• Teach patient and family self-care techniques.
• Work with interdisciplinary team.
• All other duties as assigned.
