Job summary

Bristol Hospice is hiring a compassionate Registered Nurse (RN) to perform comprehensive patient and family assessments and deliver skilled nursing care in a hospice setting. The role emphasizes the nursing process, care planning, and collaboration with an interdisciplinary team.

Applicants should have an active RN license, at least 1 year of acute care experience, and a willingness to travel across the service area to support patients and families with compassion and dignity.

Qualifications

• Must have at least 1 year of acute care experience in an institutional setting.
• Current RN licensure in the state.
• Willingness to travel throughout the Bristol Hospice service area.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments.
• Provide a complete physical assessment and history of illness(es).
• Develop and implement an evidence-based care plan with measurable goals.
• Regularly re-evaluate patient and family needs and revise the plan as necessary.
• Communicate with physicians about patient needs and orders.
• Educate patient and family on self-care techniques.
