Job summary

Bristol Hospice in the United States seeks a compassionate Registered Nurse (RN) to perform comprehensive patient and family assessments, develop plans of care, and provide professional nursing care in the hospice setting.

This role requires travel throughout the Bristol Hospice Service Area, collaboration with an interdisciplinary team, patient education, and adherence to hospice philosophy and standards.

Qualifications

• At least 1 year of acute care experience in an institutional setting.
• Home care experience preferred.
• Current state RN license.
• Willingness to travel Bristol service area.
• Knowledge of hospice philosophy and patient-centric care.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care and develop a care plan.
• Coordinate with interdisciplinary team and revise plan as needed.
• Educate patient and family on self-care techniques.
• Document care and communicate changes to physicians.
