Job summary
Bristol Hospice Llc is seeking a compassionate Registered Nurse (RN) to perform comprehensive patient and family assessments in a hospice setting. You will provide professional nursing care, develop and update care plans, and collaborate with physicians and the interdisciplinary team to support patients and families at the end of life.

Qualifications
- Minimum 1 year of acute care experience in an institutional setting.
- 1 year experience in home care preferred.
- Must hold a current Texas RN license.
- Familiarity with hospice philosophy and end-of-life care.
- Willingness to travel across Bristol Hospice service area.
Responsibilities
- Perform initial and ongoing comprehensive patient/family assessments.
- Complete physical assessments and document medical history.
- Develop and revise care plans based on nursing diagnoses.
- Record pain and symptom management and outcomes.
- Coordinate with physician and healthcare team on changes.
- Educate patients and families on self-care and care options.
- Collaborate with interdisciplinary team to deliver holistic care.
- Carry out other duties as assigned.
