Qualifications

• Must be a Registered Nurse licensed in the state of operation.
• Nursing experience required, with preference for Hospice or Home Health.
• Sound knowledge of nursing principles and patient family teaching.

Responsibilities

• Assess, monitor, and document patient's symptoms using required software.
• Collaborate with the patient and interdisciplinary team to create care plans.
• Serve as patient advocate, providing information on rights and responsibilities.
• Evaluate patient's status and educational needs for caregivers.
• Supervise aides and licensed practical nurses as per guidelines.
• Participate in an on-call rotating schedule as needed.
