Qualifications

• Must hold a valid RN license in the state of operation.
• Hospice or home health experience preferred.
• Solid nursing knowledge and ability to teach patients and families.

Responsibilities

• Assess, monitor, and document patient symptoms per physician orders.
• Collaborate with patient, family, and interdisciplinary team to develop individualized plans of care.
• Advise patients on rights, procedures, and responsibilities; advocate for patients.
• Observe and evaluate patient status, safety, and education needs for caregivers.
• Supervise and direct aides and licensed practical nurses per guidelines.
• Participate in on-call rotation and provide back-up support as needed.
