Job summary

Bristol Hospice in Utah County is seeking a compassionate Registered Nurse to perform comprehensive patient and family assessments, develop and update plans of care, and deliver skilled nursing care in a hospice setting.

You will collaborate with an interdisciplinary team, educate patients and families on self-care, coordinate with physicians, and travel throughout the Bristol Hospice service area to provide high-quality end-of-life care, with a supportive, value-driven culture.

Qualifications

• Must have at least one year of acute care experience in an institutional setting.
• One year of home care experience preferred.
• Current licensure as a Registered Nurse in Utah.
• Understand hospice philosophy and care needs for the terminally ill.
• Must be flexible in work hours and willing to travel the Bristol Hospice service area.
• Willing to maintain comprehensive knowledge of information systems and software used.

Responsibilities

• Complete initial, comprehensive assessments of patient and family to determine hospice needs.
• Provide complete physical assessment and history of current and prior illnesses.
• Assess status and initiate/Revise care plans with the interdisciplinary team.
• Record pain and symptom management, communicate changes to physician.
• Educate patient and family on self-care techniques and care expectations.
• Collaborate with the interdisciplinary group to meet patient goals.
