Job summary

Comfort Hospice is seeking a dedicated Hospice RN to serve patients in their homes and communities in the Pahrump area. You will act as a trusted Case Manager, delivering compassionate care that supports patients and families through difficult decisions with dignity and comfort.

You will complete assessments, develop individualized care plans, administer medications, and collaborate with an interdisciplinary team.

Qualifications

• Current RN license in the State of Nevada.
• Minimum of 2 years of nursing experience, hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred, not required.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient-centered care.
• Ability to work independently and a passion for making a real difference.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team that shares your commitment to excellence.
• Participate in holiday on-call rotation as needed.
