Emblem Hospice in Arizona is seeking a compassionate Hospice RN to act as a trusted Case Manager, delivering in-home care and supporting families in making care decisions with dignity and comfort. You will travel to patients’ homes across the North Phoenix andScottsdale areas, guiding care planning and collaborating with the care team.

The role requires an AZ RN license, at least 2 years of nursing experience (hospice/home health preferred), and a valid driver’s license with a reliable vehicle.

Qualifications

• Currently licensed as a Registered Nurse (RN) in Arizona.
• Minimum of 2 years of nursing experience; hospice or home health preferred.
• Valid driver’s license and reliable, insured vehicle.

Responsabilités

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.

Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Arizona
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
