Job summary

Comfort Hospice is seeking a Hospice Registered Nurse (RN) for PRN, per diem care across the Pahrump area. As a trusted Case Manager, you will deliver in-home nursing, guide families through care decisions, and provide comfort, dignity, and compassionate support in life’s most tender moments.

You will work with an interdisciplinary team committed to excellence, collaborate on care plans, and participate in on-call rotations, while prioritizing patient- and family-centered care at every visit.

Qualifications

• Current RN license in the State of Arizona.
• Minimum of 2 years of nursing experience, hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred, not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team that shares your commitment to excellence.
• Participate in on-call rotation as needed.

Requirements

• Current RN license in the State of Arizona
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver's license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference
