Qualifications

• RN with current licensure to practice professional nursing in DC.
• BSN degree preferred or 1 year nursing leadership experience.
• Hospice/home care experience and supervisory background preferred.
• Complies with professional standards and CPR certification.

Responsibilities

• Receives and reviews case referrals to determine hospice/home care needs.
• Assigns hospice personnel to cases and consults Medical Director on eligibility.
• Guides clinicians to improve care quality and ensures continuous operating hours.
• Reviews diagnosis, prognosis, medications and clinical course.
• Manages medical records and EMR workflow.
• Assists in establishing goals and plans of care with teams.
• Attends case conferences to coordinate interdisciplinary care.
• Performs quarterly record reviews and communicates findings.
• Assists in screening and interviewing new employees; supports orientation.
• Supports education programs and organizational policy development.
• Promotes adherence to standards and participates in QA/QI activities.
• Engages in community health planning and public relations activities.
