Job summary

Continuum-Care-of-Washington is seeking a Clinical Manager to coordinate patient care and supervise clinical staff in a hospice/home care setting in Poulsbo, WA. You will ensure services are delivered appropriately and work closely with the Medical Director.

The ideal candidate holds an active RN license with a BSN preferred, and has hospice/home care and management experience, strong communication and leadership skills, and proficiency with EMR systems and care planning.

Qualifications

• RN with current licensure to practice in WA.
• BSN preferred or 1 year of professional nursing in the last 3 years.
• Hospice/home care experience and 2 years of management or supervision.
• Complies with professional standards and CPR certification.
• HCHB and PointCare experience preferred.
• Excellent observation and communication, hospice philosophy understood.

Responsibilities

• Coordinate patient care and supervise clinical personnel.
• Review cases, assign hospice staff, and consult with Medical Director.
• Maintain records and EMR workflow; support care planning.
• Attend case conferences and promote quality home care services.
• Assist in interviewing and orienting new staff; support policy development.
• Participate in quality improvement and organizational initiatives.
