Job summary

Dierksen Hospice is seeking a Registered Nurse with strong clinical knowledge and supervisory experience to lead an amazing team of clinicians as their Clinical Director. The role involves guiding RNs, Aides, Social Workers, and Chaplains, collaborating with the Program Director, Hospice Physician, and Community Liaisons to ensure excellent patient care and regulatory compliance.

The ideal candidate will have a BSN, state RN licensure, and at least 3 years of nursing experience including hospice

Qualifications

• Bachelor’s degree in nursing or health care preferred.
• Registered Nurse with a current state license to practice as a professional nurse in the state the office is located.
• Minimum 3 years as a nurse, including 1 year direct patient care in hospice/home health/oncology, and 1 year supervisory experience in hospice or home health care preferred.
• Demonstrated ability to manage terminal care with crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
• Demonstrated ability to provide supportive, sensitive staff and patient/family care.

Responsibilities

• Coordinate and oversee a spectrum of patient services, ensuring adherence to hospice program policies.
• Provide guidance and support to an interdisciplinary team of clinicians.
• Conduct chart audits and ensure medical record compliance with regulations and standards.
• Participate in orientation and in-service training for nursing and clinical staff.
• Evaluate and uplift clinical staff performance during probation and annually.
• Lead interdisciplinary team meetings to refine Care Plans and foster professional collaboration.
