Job summary

Continuum Hospice in Massachusetts seeks a Registered Nurse with supervisory experience to serve as Clinical Director, leading an interdisciplinary team of RNs, Aides, Social Workers, and Chaplains. You will collaborate with the Program Director and Hospice Physician, ensure policy adherence, drive patient growth, and maintain compliance with federal and state guidelines.

Schedule is Mon-Fri 8:00 am-4:30 pm in the Braintree territory, with responsibilities to oversee care plans and assure

Qualifications

Bachelor's degree in nursing; RN license in state.
Minimum 3 years nursing experience with 1 year hospice/home health/oncology.
At least 1 year of supervisory experience in hospice or home health.
Experience in crisis intervention, pain and symptom control, bedside nursing, and family counseling.
Proficiency with computer/EMR programs.
Current CPR certification as required by state.
Valid driver's license and reliable transportation.

Responsibilities

Oversee a spectrum of patient services and ensure policy alignment.
Provide guidance to an interdisciplinary team of clinicians, fostering continuous improvement.
Conduct chart audits to ensure compliance with regulations and ethical standards.
Develop orientation and in-service training for nursing and clinical staff.
Evaluate staff performance and provide coaching to enhance care delivery.
Lead interdisciplinary team meetings to refine patient/family care plans.
