Job summary

Good Samaritan Hospice is seeking compassionate Registered Nurses to join our team as Hospice RN Case Managers, delivering end-of-life care and support to patients and families in the comfort of home settings.

As an integral member of our interdisciplinary team, you will coordinate with physicians, social workers, spiritual care, and volunteers to ensure comfort, dignity, and quality of life throughout the hospice journey. This is a full-time role with one weekend per month and on-call rotation.

Qualifications

• Current RN license in PA.
• Strong clinical assessment and nursing skills.
• Excellent written and verbal communication skills.
• Excellent computer and documentation skills.

Responsibilities

• Provide end-of-life care and support in patients' homes.
• Collaborate with physicians, social workers, spiritual care, volunteers, patients and families.
• Coordinate care as an RN case manager within an interdisciplinary team.
• Participate in weekend duties and on-call rotation.

Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking compassionate Registered Nurses (RNs) to join our team and provide exceptional end-of-life care and support to patients and their families in the comfort of their own homes.

We are currently hiring Hospice Registered Nurse Case Managers in the following areas:

• Beaver County (lower)
• Moon Township / McKees Rocks / South Hills
• Monroeville/Cheswick/Oakmont areas
