Job summary

Morning Star Hospice and Palliative Care is seeking a Registered Nurse (RN) Hospice Case Manager to join the team in the Harrisburg, PA area. The RN will manage and coordinate patient care in homes, SNFs, or assisted living facilities, working with an interdisciplinary team to assess needs and develop care plans.

Responsibilities include conducting assessments, managing medications and symptoms, educating families, and ensuring comfort and dignity at end of life.

Qualifications

• Current and unrestricted RN license in Pennsylvania.
• At least 1 year of hospice or palliative care experience preferred.

As an RN Hospice Case Manager, you will be responsible for managing and coordinating the care of patients in their homes, skilled nursing facilities, or assisted living environments.
