Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking compassionate Registered Nurses (RNs) to join our team and provide exceptional end-of-life care and support to patients and their families in the comfort of their own homes.

We are hiring Hospice Registered Nurse Case Managers in Beaver County (lower), Moon Township / McKees Rocks / South Hills, and Monroeville/Cheswick/Oakmont areas. Full-time positions with one weekend per month and on-call rotation are available.

Qualifications

• Current RN license in Pennsylvania.
• Strong clinical assessment and nursing skills.
• Excellent computer and documentation skills.
• Strong organizational skills and attention to detail.
• Excellent written and verbal communication skills.

Responsibilities

• As a Hospice RN Case Manager, you will work with the interdisciplinary team to ensure comfort, dignity, and quality of life for patients.
• Coordinate care with physicians, social workers, spiritual care, volunteers, patients, and families.
• Provide 1:1 patient care and participate in on-call rotation and weekend coverage.
