Ascend Hospice & Palliative Care PA is seeking an RN Case Manager to support on-call and case management, with visits as needed. The role involves initial patient assessments, care planning, and coordination with the healthcare team to ensure continuous, high-quality end-of-life care.

The ideal candidate holds an Associate's Degree in Nursing, an active RN license, and at least two years of nursing experience, with hospice exposure preferred.

Qualifications

• Must hold a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to provide continuous, well-coordinated care.
• Counsel the patient and family in meeting end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with the physician regarding patient needs and changes in condition.
• Supervise the Hospice Aides.

Now Hiring – RN Case Manager –

Hours: M-F 8AM-5:30PM

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:
