Qualifications

• Verifiable hospice nursing experience is strictly required.
• Active RN license in good standing.
• Permanent, long-term employment is sought.
• Clean driving record with reliable transportation for patient home visits.

Responsibilities

• Coordinate and oversee the complete hospice care plan for a caseload of patients.
• Manage complex symptoms, administer medications, and perform skilled nursing interventions.
• Collaborate with physicians, social workers, spiritual counselors, and home health aides to deliver holistic care.
• Maintain timely electronic charting of patient status and care plans.
• Educate and support families and caregivers throughout the care process.
