Job summary

Advocate Health seeks a registered nurse to join the hospice team in Wisconsin. You will coordinate patient/family care, participate in IDG, and ensure end-of-life comfort under the guidance of the Case Manager. This on-site role requires 1–2 years of nursing experience in hospice or related fields, BLS, and Wisconsin RN licensure.

Regular schedule Mon-Fri 0800-1630, 40 hours/week, with on-call rotation; no PMs or weekend requirements. On-site benefits and total rewards are offered.

Qualifications

• RN license issued by the state; BLS for Healthcare Providers certification required upon hire.
• Completion of an accredited nursing program.
• Typically 1-2 years of nursing experience in home health, hospice, medical/surgical, long-term care, emergency care/ICU or oncology.
• Excellent communication, interpersonal, and organizational skills.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients in hospice settings.
• Educate patients and families on disease processes, hospice philosophy, and end-of-life care.
• Review caseload to ensure appropriate hospice level of care and coordinate with IDG.
• Supervise home health aides and licensed practical nurses per agency standards.
• Document in electronic health records per standards and policies.
