Job summary

Resource 1 Home Staffing seeks a Hospice Registered Nurse (RN) Case Manager in Mequon, WI. This full-time role covers a hybrid schedule of facility and home-based hospice care. The RN will assess patients, plan and coordinate end-of-life care with families, and participate in on-call duties.

Qualifications

• Graduation from an accredited school of nursing.
• 1-2 years of home care or acute care experience is preferred.
• Current RN license in good standing.

Responsibilities

• Perform initial and ongoing comprehensive patient assessments and plan care.
• Provide professional nursing care and administer prescribed medications.
• Develop and revise care plans with patient and family involvement.
• Coordinate patient care across the hospice interdisciplinary team.
• Document care, communicate with physicians, and participate in on-call duties.
