• Maintain timely electronic charting of patient status and care plans.

• Starting Rate: From $44.00 per hour (Highly negotiable; starting pay is greatly influenced by your depth of experience).

We are seeking a permanent, dedicated Hospice RN Case Manager to join our compassionate care team in Milwaukee, WI. In this role, you will provide end-of-life care, symptom management, and emotional support to patients and their families. Because our service area spans counties on both sides of the state line, this position uniquely services communities in both Waukesha and Milwaukee WI.
