Job summary

UnitedHealth Group's Casa de la Paz Hospice is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients, performing initial evaluations and coordinating with the care team.

You will develop care plans, monitor progress, educate patients and families, and ensure compassionate, high-quality care. A $5,000 sign-on bonus is offered for full-time employment, with a comprehensive benefits package.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.
• 1+ years of clinical experience (preferred).
• Current CPR certification or ability to obtain within 90 days (preferred).

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family physical, psychosocial, and environmental needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care with the care team.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family, physician, and IDG members for daily care.
• Instruct and supervise self-care techniques for patients/families when appropriate.
• Maintain accurate clinical notes on patient condition.
• Work in diverse home settings regardless of background.
