Job summary

Crescent Hospice in Augusta, GA is seeking a Full Time Hospice RN Case Manager to deliver hands-on hospice nursing, coordinate services, and guide patients and families through every step of their journey.

You will collaborate with an interdisciplinary team, leverage EMR tools, and participate in after-hours call coverage. A current RN license, 1 year med-surg experience, CPR, and a valid driver's license are required.

Qualifications

• Active RN license in good standing.
• Minimum 1 year RN experience in med-surg/acute care; hospice a plus.
• Proficient with computers and EMR systems.
• Current CPR certification as required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess patient eligibility.
• Support patients and families through care planning and execution.
• Deliver education and advocacy for personalized plans.
• Participate in call coverage as part of the team.
