Job summary

Crescent Hospice is seeking a Full Time Hospice RN Case Manager to join our nursing team, delivering hands-on clinical care, coordinating services, and guiding patients and families through the hospice journey in the Augusta territory.

The ideal candidate holds an active RN license, at least one year of medical-surgical experience, EMR proficiency, CPR where required, and reliable transportation. We offer a competitive salary, comprehensive benefits, and a structured onboarding program.

Qualifications

Hold current unencumbered RN license.
Minimum one year experience as an RN in medical surgical/acute care; hospice experience a plus.
Must be computer proficient and comfortable with EMR systems.
Maintain CPR certification if required by state.
Have a valid driver's license, insurance, and reliable transportation.

Collaborative Care Champion: work with Clinical Director to assess client eligibility for hospice services.
Nurturing Direct Care: provide hands-on nursing care and ensure patient comfort and quality of life.
Nursing with Heart: deliver top-notch care grounded in proven nursing principles.
Educator and Advocate: educate patients and families and help create personalized care plans.
Call of Compassion: participate in call duties to support care coverage when needed.
