Job summary

Continuum Care Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to deliver hands-on hospice nursing care and coordinate services across South Boston, Braintree, Dorchester, Quincy, Randolph, Milton and Weymouth. This role requires an active RN license and strong EMR skills, with a focus on compassionate, patient-centered care.

Qualifications

Hold current unencumbered RN license.
Minimum 1 year RN in medical-surgical/acute care; hospice exp is a plus.

Responsibilities

Provide hands-on hospice nursing care and coordinate services.
Collaborate with Clinical Director to assess eligibility for hospice services.
Educate and advocate for patients and families to create personalized care plans.
Deliver compassionate direct care to maintain comfort and quality of life.
Participate in call duties as part of the team.
