Job summary

Caring Hospice Services is seeking a Full-Time RN Case Manager in Massachusetts to deliver nursing care with emphasis on pain and symptom control for hospice patients, implement physician plans of care, and establish a nursing plan of care based on nursing processes. You will train, supervise, and support families in patient care.

The role includes assessing patients, planning care, independent decision-making, ongoing communication with physicians, and documenting interventions.

Qualifications

• Licensed as an RN in MA.
• Hospice experience preferred.
• Knowledge of Medicare/Medicaid requirements.
• Ability to relate and treat terminally ill patients and families.

Responsibilities

• Assess the physical and psychological status of the patient.
• Reviews past and present medical history for planning.
• Provides emotional support to caregivers.
• Work independently and make critical decisions based on patient information.
• Ongoing assessment of patient and family needs for care.
• Maintain communication with prescribing physician for med changes.
• Document patient condition and nursing interventions.
