Job summary

Holisticare Hospice is seeking a Full Time Hospice RN Case Manager to join our compassionate nursing team delivering expert end-of-life care across Middlesex County with some coverage in Monmouth County. This role emphasizes hands-on nursing, care coordination, and family guidance within an interdisciplinary team.

Requirements include an active RN license, at least 1 year of clinical experience, EMR proficiency, CPR certification, and a valid driver's license. A $5,000 sign-on bonus is offered.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical-surgical/acute care setting; hospice experience a plus.
• Must be computer proficient in typing and various programs, including EMR background.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and dependable transportation.

Responsibilities

• Deliver hands-on hospice nursing care as an RN Case Manager.
• Coordinate services and guide patients and families through the hospice journey.
• Respond to referrals and inquiries with compassionate, expert service.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide education and advocacy to patients and families; participate in call duties.
