Job summary

Amedisys, part of the Optum family, is hiring an RN Case Manager to coordinate compassionate hospice care. You will assess patients, collaborate with the interdisciplinary team, and develop care plans that address each patient’s needs, with on-going communication to the care team and patients’ families.

The role requires an active RN license, CPR certification, at least 1 year of nursing experience, and a valid driver’s license.

Qualifications

• Current RN license in the state(s) of assignment.
• Current CPR certification.
• 1+ years of nursing experience.
• Valid driver's license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for non-referral.
• Develops and maintains patient care progress records per policies.
• Records pain/symptom management changes and outcomes.
• Teaches patients, families, and caregivers appropriate care techniques.
• Communicates with the clinical manager and care team.
• Performs on-call responsibilities for patients/families.
• Other duties as assigned.
