Job summary

Trinity Health is seeking a Hospice RN Case Manager to provide professional nursing care to hospice clients, develop care plans, and coordinate with the interdisciplinary team. The role includes a 15K sign-on bonus and a 36-hour workweek (Friday–Sunday, 8am–9pm) with compensation for 40 hours.

The Hospice RN Case Manager will assess, plan, implement, and supervise patient care in accordance with state nursing practice acts and agency policies, ensuring quality outcomes and caregiver support.

Qualifications

• Graduate of an approved Nursing education program.
• Current RN licensure in the state of practice.
• Minimum of 1 year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.
• Exhibits strong communication and interpersonal skills, with a special emphasis on grief/loss management.
• Must have current Driver's license and reliable transportation to and from work site.

Responsibilities

• Informs client/caregiver about agency, care/services, and Hospice benefits.
• Develops and implements Plan of Care focused on outcomes to improve quality of life.
• Makes referrals for evaluation/care and coordinates with the interdisciplinary team.
• Uses nursing judgment to assess clients and plan interventions.
• Reports changes in client condition promptly to physician/case manager.
• Re‑evaluates and updates the plan of care based on progress toward goals.
• Educates patients and families on disease, end-of-life care, and self-care.
• Documents observations and care activities in the agency system.
• Ensures timely submission of client data to the office for records.
