Job summary

Trinity Health At Home is seeking a Hospice RN Case Manager to provide professional nursing care to hospice clients as prescribed by a physician. You will assess, care plan, and supervise client care, coordinating with the IDT to ensure high-quality outcomes.

The role requires an active Michigan RN license, 1 year of acute care experience, and preferred home health/hospice background. A 15K sign-on bonus and comprehensive benefits are offered.

Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in Michigan.
• Minimum of one year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.

Responsibilities

• Provide professional nursing care to hospice clients as prescribed by the physician and in accordance with policies.
• Develop and implement an individualized Plan of Care focused on outcomes to improve quality of life.
• Coordinate care with other disciplines and services to ensure effective care.
• Document observations, interventions, and evaluations accurately in the client record.
• Communicate changes in patient condition promptly to physicians and the care team.
