Job summary

Trinity Health At Home is seeking a Hospice RN Case Manager to provide professional nursing care to hospice clients as prescribed by physicians and organizational policies. The role includes case management, care planning, symptom management, and collaboration with the IDT to optimize patient outcomes in Livingston County, Michigan.

Schedule is Friday through Sunday, 8am to 9pm, with 36 hours of work but paid for 40 hours.

Qualifications

• Graduate of an approved Nursing education program.
• Current Registered Nurse licensure in the state of practice.
• Minimum of one (1) year experience as a professional acute care nurse.
• Home healthcare/hospice experience preferred.
• Exhibits strong communication and interpersonal skills, with emphasis on grief/loss management.
• Must have current Driver’s license and reliable transportation to and from work site.

Responsibilities

• Informs client/caregiver about the agency, care/services, and patient responsibilities before care begins.
• Develops and implements an individualized plan of care focused on outcomes and quality of life.
• Coordinates care with other disciplines and services to ensure effective, efficient care.
• Assess and document patient needs, apply nursing judgment, and make recommendations to the physician and family.
• Re-evaluates and updates the plan of care based on patient goals and progress towards outcomes.
• Provide education to patients and families regarding disease, self-care, and end-of-life care.
