Job summary

Angels Care Hospice is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care and interact with other medical disciplines to provide professional care in the patient’s home. You will perform admission visits, determine eligibility for home care services, and develop the plan of care to be followed.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform the admission visit to the patient in the home and determine eligibility for home care services.
• Develop the plan of care to be followed.
