Job summary

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of home-based care. You will interact with other medical disciplines as needed, providing professional nursing care and performing admission visits in patients' homes.

You will determine eligibility for home care services and develop the plan of care to be followed, ensuring high-quality patient outcomes and adherence to regulatory standards.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One-year's experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient's home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visits to patients in the home, determine eligibility for home care services, and develop the plan of care.
