Job summary

Athena Health Care Systems is seeking a Hospice RN Case Manager for per diem coverage. The role involves planning, organizing, and directing hospice care services to patients in their homes and communities.

Responsibilities include supervising aides, conducting comprehensive assessments, developing care plans, counseling patients and families, and coordinating care as Case Manager under the Clinical Supervisor. Connecticut RN license and CPR are required.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in Connecticut with CPR certification.
• Bachelor's degree; hospice care experience preferred.

Responsibilities

• Supervise hospice aides and provide professional nursing care by utilizing all elements of the nursing process.
• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations.
• Develop and implement care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions.
• Counsel patients and families in meeting nursing and related needs, and provide health care instructions as appropriate.
• Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload.
