Job summary

Empyrean Hospice in Augusta, GA is seeking a Registered Nurse Case Manager to plan, organize, and direct hospice care with emphasis on community health education and compassionate support for patients and families.

You will assess patients, develop individualized care plans, coordinate services for your caseload, counsel families, maintain accurate clinical notes, and collaborate with physicians and the care team to ensure continuity of care.

Qualifications

• Current licensure in State and CPR certification.
• Graduate of an accredited school of nursing with 1-2 years of acute care experience.
• 1-2 years of home care experience preferred.
• Bachelor’s degree; hospice care experience preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family.
• Develop individual care plans with goals involving patient and family.
• Coordinate care for assigned caseload with the care team.
• Counsel patient and family in nursing needs and care instructions.
• Assist with activities of daily living and encourage self-sufficiency.
• Submit accurate clinical notes and communicate with physicians.
• Provide safe environment and supervise aides when required.
