Job summary

Hearth Hospice in Savannah, GA is seeking a Per Diem Hospice RN Case Manager to join our caring nursing team. You will deliver hands-on hospice care, coordinate services, and guide patients and families through the hospice journey in the Savannah area.

This role requires an active RN license, 1+ year in acute care, EMR proficiency, CPR if required, and reliable transportation. Per diem schedule Mon-Thu with opportunities for professional growth and a supportive team environment.

Qualifications

Current unencumbered Registered Nurse license.
Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
Proficient computer skills, typing, and EMR experience.
Current CPR certification if required by state.
Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

Provide hands-on hospice nursing care and coordinate services.
Collaborate with Clinical Director to assess client eligibility and suitability for hospice services.
Deliver direct patient care and support quality of life and comfort.
Apply nursing skills to ease pain and provide compassionate care.
Educate patients and families to participate in personalized care plans.
Share in call duties to ensure availability when it matters most.
