Qualifications

• Active RN license in good standing.
• Minimum 1 year RN experience in medical-surgical/acute care setting; hospice experience preferred.
• Proficient with EMR systems and computers.
• CPR certification current if required by state.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide comfort and support to patients and families to maintain quality of life.
• Apply nursing skills and evidence-based care to alleviate symptoms.
• Educate patients and families to participate in personalized care plans.
• Participate in on-call duties as part of the care team.
