Hospice of Virginia is hiring a RN Case Manager to support Richmond and surrounding counties. Come join our compassionate growing team of professional clinicians who make a difference in the lives of patients and families facing end of life journey.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledge of quality assessment and assurance procedures.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data and assess patient factors that impact needs and coordinate interventions with the interdisciplinary team.
• Initiate communication with attending physician and agencies to coordinate optimal care.
• Provide regular visits to re-assess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records and timely care plans.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.
