Job summary

JOL Healthcare is seeking a Hospice RN Case Manager to coordinate and deliver high-quality hospice care to patients in their homes and hospice facilities. You will develop individualized plans, manage visits, and collaborate with the interdisciplinary team to ensure compassionate, compliant care.

The role requires current RN license, nursing experience with hospice preferred, and strong assessment and communication skills.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the practicing state.
• Hospice and Palliative Care certification is preferred.
• Minimum of 2 years nursing experience, with at least 1 year in hospice or palliative care preferred.

Responsibilities

• Conduct comprehensive assessments of hospice patients.
• Develop and implement individualized care plans.
• Coordinate care with physicians, social workers, chaplains, and other team members.
• Provide skilled nursing care and symptom management.
• Educate patients and families on disease progression and care expectations.
• Document all patient interactions and care provided in EMR system.
• Participate in interdisciplinary team meetings and care planning.
• Ensure compliance with hospice policies and regulatory requirements.
• Respond to patient and family needs in a timely and compassionate manner.
