Job summary

Transitions Hospice in the United States seeks an experienced RN Case Manager to coordinate home-based patient care and guide the interdisciplinary team, stepping in when it matters most. You will perform comprehensive assessments, develop personalized care plans, administer medications per orders, and ensure accurate documentation while communicating with families and physicians.

On-call rotation is required.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Coordinate care and guide the interdisciplinary team.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.
