Job summary

Arkansas Hospice, Inc. is seeking a Registered Nurse Case Manager in Monticello, AR. This full-time role supports hospice patients through coordinated nursing care, assessments, and care planning.

The RN will work with an interdisciplinary team, manage care plans, and provide education to patients and families. A valid AR RN license and driving requirements apply; BSN preferred and patient-focused care is essential.

Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Minimum of one year nursing experience in a hospital, home health agency, hospice, or related program.
• Licensed as a Registered Nurse (RN) in Arkansas.
• Valid Arkansas driver’s license, personal automobile, automobile insurance required.

Responsibilities

• Manages patient care after admission and assignment to a designated RN Case Manager.
• Assists in the admission process as requested.
• Interprets Arkansas Hospice, Inc. policies for nursing staff and others to ensure understanding and compliance.
• Coordinates and collaborates with interdisciplinary team to manage the plan of care.
• Updates the Plan of Care to reflect patient status and changing needs.
• Facilitates effective written and verbal communication with patients/families and other providers.
• Maintains contact with attending physician and community resources.
• Monitors pain and symptom control for patient comfort.
• Rotates on-call duty to provide 24-hour service.
• Adheres to organizational policies and maintains confidentiality.
• Participates in team meetings, care planning activities and performance improvement.
