Job summary

Arkansas Hospice, Inc. seeks a Registered Nurse Case Manager to oversee clinical interventions and support for an assigned caseload of hospice patients. The role requires coordinating with the interdisciplinary team and maintaining current care plans.

The RN will ensure timely documentation, effective communication with families and providers, and adherence to Hospice policies. A valid AR RN license and driving authorized by the company are required.

Qualifications

• Graduation from an accredited school of nursing; BSN preferred.
• Minimum of one year nursing experience in a hospital, home health, hospice, or related program.
• Licensed as a Registered Nurse (RN) in Arkansas and valid AR driver’s license.

Responsibilities

• Manages patient care functions after admission and assignment to an RN Case Manager.
• Assists in the admission process as requested.
• Coordinates and collaborates with interdisciplinary team to manage the plan of care.
