Job summary

Superior Hospice in Minnesota is seeking a Hospice RN Case Manager to lead and advocate for patients in the Greater Twin Cities metro and surrounding counties. You will develop and manage individualized plans of care, while serving as the primary RN for patients and families.

You’ll collaborate with an interdisciplinary team, educate families, document in EMR, and ensure compassionate, dignified end‑of‑life care across homes and facilities.

Qualifications

• Active Minnesota RN license required.
• Hospice experience preferred or strong clinical background in palliative care.
• Excellent assessment, judgment, and independent work abilities.

Responsibilities

• Develop and manage individualized hospice plans of care for assigned patients.
• Complete comprehensive nursing assessments and ongoing case management.
• Monitor pain, symptoms, and changes in condition while promoting comfort, dignity, and quality of life.
• Serve as the primary RN and trusted clinical resource for patients, families, and caregivers.
• Educate patients and loved ones throughout the hospice journey with compassion and confidence.
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and other team members.
• Provide care in private homes, assisted living communities, and skilled nursing facilities.
• Maintain timely, accurate EMR documentation while ensuring compliance with Medicare hospice regulations.

What We're Looking For

• Active Minnesota Registered Nurse (RN) license.
• Experience in hospice preferred, or a strong clinical background in oncology, palliative care, ICU, home health, or chronic illness management.
• Strong assessment skills, sound clinical judgment, and the ability to work independently.
• Excellent communication, organization, and critical thinking skills.
• A deep commitment to dignity, autonomy, and relationship-centered care.
• Reliable transportation and willingness to travel throughout the service area.
