Job summary

Superior Hospice in Saint Paul, MN seeks an experienced Hospice RN Case Manager to lead patient care coordination across home, assisted living, and SNF settings. You will develop care plans, conduct assessments, monitor symptoms, and advocate for patients and families with an interdisciplinary team.

Qualifications

  • Active Minnesota RN license.
  • Hospice experience preferred or strong clinical background in oncology, palliative care, ICU, home health, or chronic illness management.
  • Strong assessment skills, clinical judgment, and ability to work independently.
  • Excellent communication, organization, and critical thinking skills.
  • Deep commitment to dignity, autonomy, and relationship-centered care.
  • Reliable transportation and willingness to travel throughout the service area.

Responsibilities

  • Develop and manage individualized hospice plans of care for your assigned patients.
  • Complete comprehensive nursing assessments and ongoing case management.
  • Monitor pain, symptoms, and changes in condition while promoting comfort, dignity, and quality of life.
  • Serve as the primary RN and trusted clinical resource for patients, families, and caregivers.
  • Educate patients and loved ones throughout the hospice journey with compassion and confidence.
  • Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and other interdisciplinary team members.
  • Provide care in private homes, assisted living communities, and skilled nursing facilities.
  • Maintain timely, accurate EMR documentation while ensuring compliance with Medicare hospice regulations.
