Superior Hospice in Robbinsdale, MN, seeks a Hospice RN Case Manager to provide one-on-one patient care in homes and facilities across the Greater Twin Cities Metro Area. You will manage caseloads, assess needs, and coordinate with an interdisciplinary team to ensure comfort, dignity, and quality of life.

Qualifications

  • Active Minnesota RN license.
  • Experience in hospice, oncology, ICU, palliative care, home health, or another high-acuity clinical setting.
  • Strong assessment skills, sound clinical judgment, and the ability to work independently.
  • Passion for relationship-centered, patient-focused care.
  • Excellent communication, organization, and critical thinking skills.
  • Reliable transportation and willingness to travel throughout the service area.
